The provision of personal data by means of this form is voluntary. If you do not provide sufficient information, we may not be able to process your application/request.
The personal data you provide by means of this form may be disclosed to other departments/agencies for the purposes mentioned in paragraph 1 above.
You have a right of access and correction with respect to personal data as provided for in sections 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance. Your right of access includes the right to obtain a copy of your personal data provided by this form.

As advised by the Office of the Privacy Commissioner for Personal Data, Hong Kong (PCO), a prescribed form for the data access requests should be used with effect from 1 December 1999. From 1 December 1999 onwards, data users have the discretion to refuse any data access requests not put forward by means of the prescribed form. Data subjects who wish to lodge their requests could obtain the prescribed form both from District Offices and the PCO. Copies of the form can also be downloaded from the PCO website at http://www.pco.org.hk
But for the data corrections, no specified form is requested. Following the supply by a data user of a copy of personal data, the data subject is entitled to ask for correction on inaccurate data. If you wish to lodge a data correction, you could submit your request in writing to our Personal Data Privacy Officer in compliance with the Personal Data (Privacy) Ordinance.
In this interim period, this department will still accept any data access/correction requests made under Personal Data (Privacy) Ordinance by means of the form as shown in the page headed with "Enquiries & Addresses" despite the form therein is not designed for such purpose. With effect from 1 December 1999, only those requests made under the Code on Access to Information will be entertained if requestors use the form in the page headed with "Enquiries & Addresses".
